Ask at the consultation
- □ Which structure causes the concern I notice?
- □ What can this procedure change—and what will remain?
- □ What alternatives, including observation, fit the concern?
- □ What are the risks, recovery plan, credentials, facility, and full written cost?
Check the decision process
- □ Have I understood the proposed procedure in my own words?
- □ Have I seen representative examples with comparable starting concerns?
- □ Would a second opinion help?
- □ Do I feel free to wait or decline?
Do not use an elective worksheet for acute symptoms
